I see a lot of children and teens in my practice with behavioral or mental health disorders that are prescribed medications known as atypical antipsychotics. They’re called “atypical” because they are much less likely to cause the “typical” side effects of first generation antipsychotics medications that some people likened to “zombie-like” behavior. While these medicines can have dramatic effects on a person’s mood and behavior, one significant side effect of many of these medicines has been weight gain, increased blood sugar levels, high blood pressure and even diabetes. Higher levels of fat and cholesterol are also possible complications.
Commonly prescribed atypical antipsychotics include Olanzapine (Zyprexa), Risperidone (Risperdal), Quetiapine (Seroquel), Ziprasidone (Geodon) and Aripiprazole (Abilify). In children, the most commonly prescribed of these drugs is risperidone. Two of the newest drugs, Geodon and Abilify, are thought to have fewer bad side effects. The latest to be FDA approved in 2006 is called Paliperidone (Invega).
The most common reason these drugs are used in children is for serious aggressive and disruptive behaviors. In adults, they are used for conditions ranging from severe depression to schizophrenia and bipolar disorder. This does not include attention deficit disorder, or ADD, but many children with ADD also have disruptive behavior disorders and may sometimes be prescribed these drugs. In 2007, the FDA approved Seroquel and Zyprexa to treat schizophrenia and bipolar disorder in children aged 13 to 17. A lot still remains unknown about the long term safety of these drugs in children.
Rarely do I hear from families that they were aware that weight gain, diabetes or cholesterol problems were possible side effects of these drugs. This is in spite of the fact that since 2003 the FDA has recommended that all patients on these drugs get screened annually for diabetes and abnormal cholesterol levels. Recent studies suggest this is being done in less than one third of cases.
